Concierge Service of West Volusia Family & Sports Medicine

We are striving to provide you with the best possible healthcare and make each experience as easy and convenient as possible. However, we understand that there have been request to have a higher level of access to your Doctor/Provider for emergency care and communication afer hours. We have decided to implement a Concierge Service to meet these needs.

Please be aware that you do not need this service to receive the best possible care at our office. This program only looks to offer additonal optons to meet the expectatons of patents who wish to purchase this service.

Specifics of the program:

• Emergency same day appointments - Our goal is to get you in with whomever you picked as your Concierge provider. However, there are times when they are not available. During those times you will still be able to receive a same day appointment with another available provider.
• Additional paperwork completed (Disability, Parking) and Prior Authorization
• Convenient laboratory collection in our lab at 1639 N Volusia Ave Orange City, FL (386) 777-2273
• New prescriptions and prescription refills sent electronically to the pharmacy (except controlled substances).
• Discounts to Aesthetic services.
• The cost is $3,000.00 annually from the date of payment.
